Instructions for Printing Medical Release Forms in e-AYSO.
Each player must provide two (2) signed medical release forms to the coach or team parent before practicing or playing in a game. Please follow these instructions to print the forms.
Access the e-AYSO website
· Login with your email address and password (If you don’t know this information, contact Kate Gibson at kgibsonayso@socal.rr.com . PLEASE DO NOT CREATE ANOTHER EAYSO ACCOUNT OR CLICK ON ""Forgot Email?" or "Forgot Password?" )
· You will then find a Welcome screen with your name.
· Click on the Print player form in the left column.
· Click on the printer icon beside your player’s name (Turn you Pop-Up Blocker off)
· Your child’s Medical Release/Registration Form will come up on the screen in the background. Read the message and click “OK”
· Click on the printer icon with the red arrow pointing at it. (You only need to print the 1st page of the document)
· Print the form twice.
· Sign the forms
· Hand the forms to the coach or team parent.
