Coaches will be requiring each parent to submit at least TWO signed Medical Release forms to their first practice.

Your child's player registration form IS the Medical Release Form.  To download this pre-printed form, you may return to eAYSO and log in with your user email address and print your child's registration forms x 2...sign and date and give to your coach.

OR

If you are having difficulty accessing the form on eAYSO, you may go to the Player & Parents tab at the LEFT hand column of our website and download the BLANK player medical release form x2 and hand-write the information and sumbit this to your coach.
